Erectile Dysfunction (ED)

ED or Erectile dysfunction is sexual dysfunction, it is characterized by inability to maintain or develop the erection of penis sufficiently for a satisfactory sexual performance. It occurs as hydraulic effect due to entering in blood and being retained in the sponge like body within penis. This process is often initiated as result of sexual arousal, it happens when signals are transmitted from brain to nerves in pelvis. The erectile dysfunction is usually indicated when the erection is difficult or not possible to produce, despite of arousal. Most important organic cause is cardiovascular disease and diabetes, hormonal insufficiencies, neurological problems and side effects of drugs.

There are complications and contributions, sometimes primary psychological relational problems. A notable psychological importance is erections or penetration fails due the feelings and thoughts rather than physical impossibilities. There is strong response to the placebo treatment. Tied closely as to cultural notions of potency, masculinity and success that can have serve the psychological consequences. Besides the treatment of psychological consequences and underlying causes, first line treatment consists of trial of PDE5 inhibitor drugs.

The treatment can be involved prostaglandin tablets in urethra and the intracavernous injections with fine needle in penis that causes penile prosthesis, swelling, vascular reconstruction or penis pump.

No any formal test is there to diagnose the erectile dysfunction. Some blood tests are done to exclude underlying disease, as hypogonadism, diabetes and prolactinoma.

Its importance is related to poor physical health generally, obesity, poor dietary habits and specifically cardiovascular disease as coronary artery disease and the peripheral vascular disease. Simple and useful way to distinguish between psychological and physiological importance it to determine that whether patient has ever erection, if never then problem is physiological is it has sometimes then it could be the physiological. Current statistical and diagnostic manual of diseases has included listing for importance.
